More information on our process

Step 1

Referral from Your Primary Care Provider

Your journey begins with a referral from your family doctor or primary care provider. This allows us to review your medical history and ensure the most appropriate next steps.

Step 2

Intake Call with Our Nurse

Once we receive your referral, our intake nurse will contact you and your support person to:

  • Review symptoms and concerns
  • Gather additional medication history and supporting information
  • Send you a self-assessment questionnaire
  • Explain what to expect during your assessment

This call helps ensure your visit is tailored to your needs.

Step 3

Meet with the Intake Nurse

You and your support person will meet a nurse who has expertise in dementia, movement disorders, and mental health disorders. This visit may include:

  • A detailed interview
  • Questionnaires
  • A review of prior imaging, history, medications, and lab results

We take the time to understand your symptoms, goals, and concerns.

Step 4

Additional Testing (Alzheimer’s only)

Based on the above results, our team may recommend additional testing, such as:

  • Genetic testing (APOE)
  • Alzheimer’s biomarkers (p-tau 217)
  • Brain MRI
  • PET imaging
  • Cerebrospinal fluid (CSF) analysis

Not every patient will need these tests. Recommendations are made on an individual basis.

Step 5

Review Results with a Specialist

You will meet with a specialist to review your results in detail. During this visit, we:

  • Explain findings in clear, understandable terms
  • Discuss what the results mean for you
  • Review diagnosis and next steps

You will have time to ask questions and discuss options including available treatment options and clinical trial opportunities.

Current Options for Alzheimer’s Disease

For qualifying patients, different options may be offered. These may include:

  1. Leqembi (currently available)
  2. Donanemab (upcoming 2026)
  3. Acetylcholinesterase inhibitors such as Aricept (donepezil)
  4. Clinical trials for new treatment options

We work closely with you and your care team to determine the most appropriate plan.

Patient-Centered, Specialist-Led Care

Our goal is to provide clarity, support, and access to advanced Alzheimer’s care in a setting that respects both patients and their families. Every step is guided by specialists, tailored to the individual, and is optional based on you and your preference. We help provide a clear and smooth path to learning more about Alzheimer’s disease and the journey ahead.
